What to Expect During Your Exam

  • Review of medical history
  • Blood pressure and vital signs check
  • Vision and hearing examination
  • Urinalysis screening
  • Review of insulin management and glucose control (if applicable)
  • Review of completed MCSA-5870 form (if applicable)
  • Issuance of DOT medical certificate if qualified

Required Forms

  • MCSA-5875 (Medical Examination Report):
    Clients are encouraged to complete the health history section prior to their visit. This includes medical history, medications, and current conditions.
  • MCSA-5870 (Insulin-Treated Diabetes Mellitus Assessment Form):
    Required for drivers who use insulin. This form must be completed by your treating clinician within 45 days of your exam.

What to Bring to Your Appointment

  • Valid driver’s license or photo ID
  • Glasses or contact lenses (if used)
  • Hearing aids (if used)
  • List of medications and dosages
  • Treating clinician’s name and contact information
  • Recent medical records for ongoing conditions (if applicable)

Additional Documents (If Applicable)

  • MCSA-5870 form (for insulin-treated diabetes)
  • Cardiac clearance letter (for heart conditions)
  • Sleep apnea compliance report (CPAP usage printout)
  • Vision exemption or waiver
  • Detailed medication list

Important Information for Diabetic Drivers

If you use insulin, you must have your treating clinician complete the MCSA-5870 form within 45 days of your exam. To help ensure a smooth process, it is also recommended to bring:

  • A1C results
  • Blood glucose logs (last 3 months)
  • Insulin regimen details
